In this guide, we’ll walk through how to create a questionnaire, configure its settings, and understand how the respondent experience works.
Overview
The Questionnaires section in Formalize allows you to create fully customizable questionnaires from scratch, or use our pre-built templates such as Business Impact Assessments or Employee Training forms. You can collect responses from both internal users, like employees, and external parties, including suppliers or third-party stakeholders. For additional security, you can enable authentication to control access.
All responses are centralized within the specific questionnaire, allowing you to track progress in real time.

How to Add a Response
In addition to sharing a public link to the questionnaire, you can also invite specific individuals to respond directly.
Add a Response Manually

Open the questionnaire where you'd like to invite someone.

Click "+ Add Response".

Enable "Invite respondents", enter the respondent's name and email, then click "Create".

You’ll now see the person listed among the respondents, along with their response status and progress. By clicking “View” on a specific response, you can access more detailed information about their answers and activity.
Respondents point of view
The invited respondent will receive an email invitation that includes a link to the questionnaire.
When they click the link, they’ll be asked to enter their email address to authenticate, based on the access settings configured in the questionnaire.

After entering their email, they'll receive a verification code via email, which they need to input to access and complete the questionnaire.
While completing your questionnaire, you can save it as a draft if you haven’t finished all the questions, you can return to it later by accessing. it again just like before. Once all questions are completed, simply click Submit to send your responses.
